Requirements
State law requires that all children have a complete physical examination -- including an eye examination for kindergarten and specific immunizations, such as a hepatitis B vaccination -- upon entering kindergarten and upon entering the sixth grade.
Sixth-grade students now are required to show proof of hepatitis B immunization. It also is now mandatory that all preschool and kindergarten children receive a chicken pox vaccination. If a child already has had the disease, parents must provide a note from a physician stating so.
Physical examinations that are not more than a year old prior to the first day of school, including those given outside the state, are acceptable. A dental exam for all students in kindergarten and second and sixth grades also is mandatory.
The school code of Illinois also requires that students who participate in interscholastic athletics have an annual physical examination.
All students are required to provide proof of residency in the form of one of the following: a mortgage contract, a real estate tax bill, a signed Department of Housing and Urban Development closing statement or a signed lease with children's names listed.
At least two of the following items also are required: a current Illinois driver's license or identification card; Illinois auto license plate registration; homeowners, apartment or auto insurance policy; Illinois Department of Public Aid Medicaid card with expiration date; most recent utility bill; voter's registration card; or Lansing vehicle license application and sticker number.
All students new to the district also must provide an Illinois transfer form from the previously attended Illinois public school. Kindergarten students and transfer students need to provide an original birth certificate.
