CALUMET CITY
Library cards available at no cost to residents
Calumet City residents are eligible to apply for a Calumet City Public Library card. The cards are free for residents.
Those interested may apply in person at the library, 660 Manistee Ave. Youths younger than 14 must have a parent or legal guardian with them to verify residency and sign the application.
A letter will be mailed within five to seven business days. Bring in the letter and envelope, along with a picture ID, to pick up the card.
Establish proof of residence with two of the following pieces of identification showing name and current address (at least one must have a photo): valid Illinois driver's license or state ID card; utility bill, dated within the past 30 days with current address; bank statement or bill dated within the past 30 days with current address; lease or mortgage papers received within the past 30 days.
The surname on the identification presented must be the same as the name on the library card. Identification with a P.O. box address will not be accepted.
Library cards are valid for one year; they must be renewed annually. Nonresident cards may be obtained for a fee.
Chicago residents with a valid Chicago Public Library card in good standing may apply for a Metropolitan Library System reciprocal borrower's card at no charge. Two pieces of identification showing Chicago residency are required.

UNIVERSITY PARK
Offerings in physical therapy subject of event
Governors State University's College of Health and Human Services will host a doctor of physical therapy and transitional doctor of physical therapy degree programs information session Oct. 29 on campus, 1 University Parkway.
Registration begins at 4 p.m. The formal program is from 4:30 to 7:30 p.m. in room D-34000. Prospective students will learn about the degree programs and can meet with current students, faculty and advisers.
Scheduled to begin in January, the transitional doctor of physical therapy program is designed for licensed physical therapists to transition into a doctoral physical therapy program.

SOUTH HOLLAND
South Suburban seeks input for self-evaluation
South Suburban College is preparing for a comprehensive site evaluation visit in February by the Higher Learning Commission.
The college is accredited by the HLC of the North Central Association of Colleges and Schools.
South Suburban is engaging in a process of self-study and evaluation to renew the 10-year accreditation that was granted after the learning commission's last visit. As part of the process, officials are seeking input from the public regarding the quality of the institution, and its services and academic programs.
College officials encourage residents of College District 510 to submit their comments in writing by Jan. 2. Comments are not confidential and must include the name, address and telephone number of the person providing them.
