DOLTON | Dolton West Elementary School District 148 has increased the number of dates parents and guardians can register their children for the approaching 2013-14 school year.

In order for students to begin school on the opening day of classes, children must be registered between 9 a.m. and 2 p.m. Aug. 7 or between 2 and 7 p.m. Aug. 13. Parents/guardians must be present to register students at the school they will attend this fall, and to provide proof of residency.

Registration for the Early Childhood Center will be at Riverdale Elementary School, 325 W. 142nd St., Riverdale. Returning students placed in ECHO special education programs outside District 148 boundaries should register at the district Administrative Office at 114 W. 144th St., Riverdale.

Parents of Early Childhood Center students can sign up for a screening appointment at registration. Screening dates are Aug. 12, 14, and 15.

The first student attendance day of the 2013-14 school year will be Aug. 20. Students in full-day kindergarten through sixth grade will attend classes from 8:45 a.m. to 3 p.m. Students in seventh and eighth grade will attend school from 8:45 a.m. until 2:30 p.m. Please note, all District 148 schools will follow this new school day start time.

The registration fee is $55 for students in kindergarten through eighth grade. All fees are payable with cash or money order. No personal checks will be accepted.

Parents and guardians of all students must show proof of residency by providing a driver’s license or state of Illinois identification card with a valid picture ID and current address, and two of the following (with name, current address, and date — within 30 days): mortgage payment book, real estate tax bill, current utility bill — electric, gas, village water bill — cable or satellite television bill, Medicaid card, KidCare identification card, current payroll or government check with address/pension check; three business letters (within 30 days) — this may include a bank statement, credit card statement, doctor bill, or insurance bill. No personal letters will be accepted.

Students moving to another address within District 148 are required to prove residency, under the new address, as a new student.

Parents/guardians of new District 148 students and those transferring in must also provide the child’s medical records, State of Illinois Transfer (ISBE 33-78), legal guardianship papers, an original birth certificate (with seal and complete birth index number), and Individualized Education Plan (IEP), if applicable.

For admission to District 148 schools, the parent or court-appointed guardian also must provide current and complete physical examination and immunization records for students entering pre-kindergarten, kindergarten or first grade, and sixth grade. Any child under the age of 6 must provide proof of at least one lead screening assessment.

A dental exam is required for students entering kindergarten, second, and sixth grade. An eye exam is required for any child entering kindergarten or an Illinois school for the first time. Students entering, advancing, or transferring into sixth, seventh, and eighth grade are required to show proof of receiving one dose of Tdap (Tetanus, Diphtheria, Pertussis) vaccine.
