The Porter County Health Department is offering immunization clinics for students in grades 6-12 needing to update recently required immunizations from 4 to 7 p.m. on Sept. 10 and 17.
State law requires the vaccine Tdap for protection against tetanus, diphtheria and pertussis; MCV4 for protection against bacterial meningitis; and varicella for protection against chickenpox (second dose).
The immunizations will be held at the county administration building at 155 Indiana Avenue Suite 104, Valparaiso.
Guidelines:
* Complete immunization record for each child being vaccinated must be brought to clinic.
* Custodial parent or legal guardian is required to sign consent to vaccinate for any child under 18 years of age.
* If custodial parent or legal guardian cannot attend event, written authorization to vaccinate by custodial parent is required. Written permission must include child's name, request to vaccinate with any needed vaccines, parent's signature, relationship to child and date.
Parents are requested not to use attached parking garage.
